Purpose

Our outreach programs focus on addressing health disparities in under-served groups.

Current programming includes health screenings for chronic hepatitis B and patient health education presentations about liver diseases for a predominantly Vietnamese immigrant population in San Jose, California. We collaborate with Song Manh Magazine, a non-profit San Jose-based medical and health magazine for the Vietnamese community, to implement annual health screening and education programs for liver-related diseases.

Our goal is to provide health screenings and patient education for any prevalent diseases in under-served communities. Such diseases include chronic hepatitis B, diabetes, high cholesterol, and hypertension.

We hope to extend our outreach efforts to Fresno and the East Bay. We welcome opportunities to collaborate with local organizations with similar interests.

In addition to health screenings and education programs, PHF can provide funding for organizations to implement community projects that align with our mission. Funding will be considered after submission of a grant proposal from the organization.
